Assessment

Cognitive Assessment:
  • A telehealth evaluation for adults noticing changes in memory, attention, or thinking - including concussion, autoimmune conditions, and other medical conditions that affect cognition. Whether the goal is to investigate a specific concern or to understand what is behind the changes you have noticed, the evaluation provides a clear picture of how your thinking skills are functioning, establishes a baseline to track over time, and offers individualized recommendations.

Adult ADHD Assessment:
  • A focused evaluation for adults questioning whether ADHD explains long-standing difficulties with attention, focus, and organization. Because these symptoms can have several causes, the evaluation looks at the full picture - combining a thorough clinical interview, validated rating scales, developmental and collateral history, records review, and cognitive measures to reach a clear formulation and practical recommendations.

  • Cognitive Rehabilitation
Specialized therapy for functional neurological and somatic conditions, including somatic symptom disorder and the anxiety that often accompanies them. These symptoms arise from changes in how the brain sends, receives, and processes signals throughout the body Using evidence-based approaches, Dr. Milam helps patients understand their symptoms and work toward meaningful improvement. Care also includes support for adjusting to a new or chronic medical diagnosis and cognitive rehabilitation for everyday memory, attention, and thinking difficulties.

Cognitive Health Consultation

A single session for patients, families, and caregivers who have questions about a cognitive or neurological diagnosis, prognosis, or treatment plan. Rather than testing, this is dedicated time to talk through what a diagnosis means, weigh the available options, and leave with clear, individualized resources and a practical plan for next steps.

Assessment Process

The Assessment Process

1

Clinical Interview

The patient will participate in a 45 to 90 minute telehealth interview.

The provider will review details of current cognitive concerns, physical symptoms, medical history, and psychosocial information to gain a holistic overview of the patient’s background and care needs.

2

Cognitive and Psychological Evaluation

The patient and provider complete a one-on-one evaluation using measures selected for the specific referral question. Depending on the concerns presented, this may include assessment of attention, processing speed, language, learning and memory, visuospatial skills, executive functioning, and neurobehavioral and psychological symptoms.

Testing typically runs 1.5 to 3.5 hours. During testing, the patient should be in a quiet, private environment to support best effort and accurate results.

3

Feedback and Report

The patient will be provided with a 30 to 60 minute feedback session reviewing their performance across tests, areas of strengths and weaknesses, clinical impressions, diagnostic and prognostic findings, and individualized treatment recommendations.

A report including all information in the clinical interview, testing, and feedback will also be sent to the referring provider with a summary sheet available in the patient portal approximately 1-2 weeks after the assessment.
