For Referring Providers:

Lumina provides telehealth neuropsychological assessment and specialty therapy for adults across Texas. Dr. Milam works to clarify the neurological, psychological, and functional processes contributing to a patient's cognitive or somatic presentation, and provides targeted treatment for functional neurological and somatic conditions.

The following are guidelines to identify patients appropriate for referral to our office.

Assessment:

  • Memory & cognitive concerns, including cognitive change related to concussion/mild TBI, autoimmune conditions, and other medical conditions, as well as differentiation of normal aging, mild cognitive impairment, and mood or anxiety-related cognitive complaints.

  • Adult ADHD diagnostic clarification, including differentiation from mood, anxiety, sleep, and other contributors.

  • Presurgical psychological/behavioral evaluations (e.g., spinal cord stimulator, transplant, bariatric) where the question is psychological readiness rather than cognitive or motor testing.

Specialty therapy:

  • Functional neurological disorder (functional seizures, movement, cognitive, and sensory/vestibular presentations), somatic symptom disorder, and associated anxiety; adjustment to a medical diagnosis; cognitive rehabilitation.

Appropriate for telehealth referral:

  • Adults (18+) physically located in Texas at the time of service.

  • Presentations suitable for remote assessment or care, with the patient able to participate by video in a private setting.

Not an appropriate referral:

  • For assessment: cases requiring comprehensive in-person testing, including presentations with significant motor or visuospatial demands; presurgical evaluations requiring comprehensive cognitive, motor, or visuospatial assessment (e.g., deep brain stimulation, epilepsy surgery); and comprehensive differential workups to distinguish dementia subtypes where the diagnostic question depends on in-person testing requiring visuospatial or motor components (i.e. Parkinson's, Lewy Body Dementia). (Evaluation to characterize cognitive impairment, establish a baseline, and guide care planning, including for MCI and mild dementia, is available.)

  • For assessment or therapy: acute or rapidly worsening presentations, or suspected acute neurological injury, which warrant urgent medical evaluation rather than scheduled services.

  • Forensic evaluations, or questions of legal capacity or competency.

Where a referral isn't suitable for telehealth, Dr. Milam triages for telehealth appropriateness and helps direct the patient toward appropriate in-person or specialized services.

What to expect:

  • The referring provider receives a concise summary letter with the full report attached, typically within 1–2 weeks of completing testing.

  • Dr. Milam follows up directly on clinical questions.

  • Lumina is private-pay and out-of-network for all insurance. Superbills are provided to patients for out-of-network reimbursement. Please advise patients of this when referring.
